Mandate Form

To set up the debit order, please email the following documents:

• Proof of payment (R12,000.00)
• Copy of Payers ID (certification not required)
• Latest three (3) months鈥� bank statements
• Signed 鈥淎uthority to Debit Account鈥� form

All documents must be emailed to your relevant course coordinator, with only your student number in the subject line.
